COVID-19 Information

Dear Patients,

Your health and the health of our staff are our highest priority.   We follow strict Infection Control protocols and Standard Precautions as recommended by the CDC, CDA, and OSHA.

For the protection of all our patients and staff due to the recent outbreak of coronavirus, we are requesting the following:

  •  We recommend you call our office and reschedule your dental appointment if you are ill,  presently have, or have had within the last 14 days any of  the following symptoms: 
    • dry cough
    • sore throat or runny nose
    • fever or chills
    • headaches or muscle aches
    • chest pain or difficulty breathing
    • reduction in your sense of smell or taste 
    • Other contagious diseases such as flu
  • Please notify our staff before your appointment if:
  •        you have been in contact with someone who has tested positive for COVID-19 in the last 14 days
  •        you have traveled more than 100 miles from your home in the last 14 days

If you are healthy, there’s no need to cancel your regularly scheduled dental appointment.

If you have an upcoming appointment, please click on the link below to print out a COVID-19 Screening Form and fill out the first column only.  You may email the form back to us at or by fax at (650)327-3103.  If we do not receive a screening form from you within 48 hours of your appointment, our office will check in with you via phone. 

Please call our office and remain in your car when you arrive for your appointment.  To comply with social distancing we may ask you to remain in your car for several minutes while we check-in/out other patients.  We thank you for your patience and flexibility during this time.
